Child Immunization Clinics

New immunization requirements for seventh grade and kindergarten students!

Effective with the 2010-2011 school year:

* All Seventh graders will be required to receive the Tdap or Td booster.

* All children entering Kindergarten will be required to have a fourth dose of polio (must be given after the 4th birthday).

* All children entering Kindergarten will be required to have two doses of varicella (chickenpox) vaccine.

  Protect yourself and your children. Keep your immunizations up to date!

Pediatric Immunization Clinics

!! NEW DAY, NEW HOURS BEGINNING JULY 2011 !!

Immunization clinics for children (birth to 18 years) are held as follows:

The first THURSDAY of each month from 8:30 am to 11:00 am at Elyria City Health District - 202 Chestnut Street

The third THURSDAY of each month from 1 pm to 4:00 pm at Elyria City Health District - 202 Chestnut Street

No appointment is necessary. Clients must sign in upon arrival and are seen on a first come - first served basis. Cost of each immunization is $14.00. Ohio Medicaid and Medicaid HMOs are accepted. Children must be accompanied by a parent or legal guardian. Parents must bring the child's immunization record. Call 323-7595 for more information.  

Immunization Schedule

In compliance with recommendations of the Ohio Department of Health, ECHD provides immunizations on the following schedule:

Birth Hep-B #1
1-2 months Hep-B #2
2 months DTaP, HIB, IPV, Prevnar, Rotavirus
4 months DTaP, HIB, IPV, Prevnar, Rotavirus
6 months DTaP, HIB, IPV, Prevnar, Rotavirus, Hep-B #3
12-15 months MMR #1, HIB, Varicella #1, Prevnar, Hep-A #1
15 months DTaP
18-24 months Hep-A #2 (6 mos after 1st dose)
4-5 years DTaP, IPV, MMR #2, Varicella #2
11-15 years TdaP
15-18 years Meningococcal
Every ten years Tetanus booster

NO child is turned away for inability to pay for vaccine.

Important: Remember to bring your child's immunization record.  Children under age 18 must be accompanied by a parent or guardian.
